Proper Identification

ID Card Requirement

You are required to have two (2) forms of official ID with a signature (one must be a photo identification). 

Note: Photocopies of identification will not be accepted.

Examples of identification you can use include:

  • Driver’s license 
  • Social Security card 
  • Clinic card 
  • Credit card
  • Library card 
  • State-issued identification card
  • Passport
  • Alien registration card 

The name on your identification must be the same as the name you used on the application to register for the examination. If you do not bring proper identification, you will not be allowed to test, and your examination fee will not be refunded. If you no show for two examinations your application to test will be removed and you will need to re-apply.

Note: A driver’s license or state-issued ID card that has been voided (has a hole punched in it) is only valid if accompanied by a letter issued by the Department of Motor Vehicles (DMV) advising your new license is being issued to you via USPS mail). If you do not have a letter issued by the Department of Motor Vehicles, the voided ID is invalid and will not be accepted as a valid form of ID. You will not be admitted for testing and you will be considered a no show. You will forfeit your testing fees and have to pay for another exam date.
