The CNA Registry

Update your name or address with the OBRA Registry promptly to stay compliant

The Alaska State Board of Nursing must be kept informed of your current address and name so that you can receive notification of certification renewal on the Registry. Failure to inform the board of an address change may risk your certification status. If your address or name changes at any time after you are placed on the Registry, you must send written notification of this change to the Alaska board. Address Change form and a copy of your legal name change document(s) (e.g. marriage certificate, court decree, etc.).

Form 08-4843 (Name Change and/or Address Change) can be found here: https://www.commerce.alaska.gov/.

Certification Renewal

Nurse aide certificates must be renewed every two years in even-numbered years regardless of when a certificate was issued. Certified Nurse Aides on the Alaska Registry can renew by paper or online through their MyAlaska/MY LICENSE account.

Visit https://www.commerce.alaska.gov/web/cbpl/ or contact the Alaska State Board of Nursing at (907) 269-8160 for questions regarding this process.

Lapsed Certification

If you do not meet the employment and continuing education requirements to renew your nurse aide certificate during the preceding renewal period or your nurse aide certificate has been lapsed for more than one missed renewal period, your nurse aide certificate will change to a lapsed status.

To change your status with the Alaska Registry from lapsed to active, you will need to complete Form 08-4365 (Application for Reinstatement of Nurse Aide Certification) and submit it to the Alaska State Board of Nursing for review. If the board determined that you would need to re-take the NNAAP® Examination in order to qualify for reinstatement, you will need to submit a new Application to Test through Credentia platform.
